Vpliv prebadanja jezika (piercinga) na ustno floro

Izhodišce V ustni votlini se med razlicnimi kovinami lahko pojavi elektricna napetost. Ob stiku dveh kovin med njima stece elektricni tok. Znano je, da imata tako elektricna napetost kot elektricni tok baktericiden ucinek. Zanimajo nas elektricne napetosti in elektricni tokovi, ki zelo verjetno nastanejo med razlicnimi amalgamskimi plombami in kirurškim jeklom, iz katerega je narejena vecina LDJ (lepotni dodatek v jeziku). Namen Ugotoviti smo želeli vpliv kovine v lepotnem dodatku na okolje v ustni votlini. Predvsem nameravamo preuciti vpliv na kariogeno ustno floro. Hipoteza Z delovno hipotezo smo predpostavili, da imajo ljudje z LDJ zaradi baktericidnega ucinka elektricnega toka in napetosti v ustni votlini manj kariogenih bakterij v primerjavi z ljudmi brez LDJ. Metode Meritve elektricnih napetosti in tokov smo za kirurško jeklo in vzorce amalgamov opravili in vitro. V testni skupini (preiskovanci z LDJ, N=17) smo in vivo izmerili elektricne napetosti in tokove med LDJ (v našem primeru kirurško jeklo) in amalgami. Pri testni skupini (preiskovanci z LDJ, N=17) in pri kontrolni skupini (preiskovanci brez LDJ, N=8) smo ugotavljali prisotnost kariogenih bakterij (Streptococcus mutans in Lactobacilli) v slini. V obeh skupinah smo preiskovance tudi klinicno pregledali. Za statisticno obdelavo podatkov smo uporabili dvosmerno korelacijo po Pearsonu s pomocjo racunalniškega programa SPSS 10.0©. Rezultati Napetosti med amalgami in LDJ pri merjenju in vitro s casom padajo, povprecje po eni uri merjenja je -161,60±115,67 mV. Tokovi prav tako padajo s casom merjenja, po 10 minutah je povprecna vrednost -0,43±0,51 µA. In vivo povprecna napetost med amalgamom in LDJ je -37,51±66,05 mV. Tokovi v ustih po stiku dveh kovin hitro padejo, po eni minuti je povprecna vrednost -0,06±0,07 µA. Preiskovanci z LDJ imajo v ustih vec streptokoka mutans (p=0,032) in vec laktobacilov (ni statisticno znacilno) kot preiskovanci brez LDJ. Prav tako imajo vecji indeks vidnega plaka (VPI) (p=0,002) in indeks krvavitve iz dlesni (GBI) (ni statisticno znacilno). Pri preiskovancih z LDJ je vec karioznih zobnih ploskev (KP) (p=0,009) in karioznih, ekstrahiranih in plombiranih zobnih ploskev (KEPP) (ni statisticno znacilno) kot pri preiskovancih brez LDJ. Zakljucki LDJ povzroci napetost in obcasno tudi tok v primeru stika med njim in amalgamskimi plombami. V nasprotju s hipotezo imajo preiskovanci z LDJ v ustih vec streptokoka mutans in laktobacilov kot preiskovanci brez LDJ. To razliko lahko povežemo z VPI ter tudi s KP, KEPP in GBI, ceprav statisticno neznacilno.

Background Inside the oral cavity the electric voltage occurs among different metals. Therefore the electric current occurs between two metals. The voltage and the current both have bactericidal effects. We are interested in electric voltages and currents which very likely occur between amalgam fillings and surgical steel of which the majority of tongue jewellery used for piercing are made. Aim Our goal was to establish the influence of the metal tongue jewellery on the oral cavity environment. Our intention is to study the influence on oral florae. Hypothesis The working hypothesis was that the subjects with tongue jewellery have less cariogenic bacteria than those without jewellery which is due to the bactericidal effect of electric voltage and the current. Methods Electric potentials, voltages and currents among surgical steel and amalgam we measured in vitro. In a test group (subjects with tongue piercing, N=17) electric voltages and currents between the tongue jewellery (in our case surgical steel) and amalgam were measured in vivo. In the test group (subjects with tongue piercing, N=17) and in the control group (subjects without tongue piercing, N=8) the presence of cariogenic bacteria in the sputum was measured. The sputum of subjects of both groups was checked for cariogenic bacteria (Streptococcus mutans and Lactobacilli). The subjects of both groups had been clinically checked. The data was analysed with the SPPS 10.0© statistical software package. Results The voltages between the amalgam and tongue jewellery measured in vitro decrease in time. The average value after an hour equals -161,60±115,67 mV. The currents decrease in time too, their average value after 10 min.equals -0,43±0,51 µA. In vivo average voltage between amalgam nad tongue jewellery is -37,51±66,05 mV. The currents in mouth decrease faster after the contact of two metals. The average value after one minute is -0,06±0,07 µA. Subjects with tongue jewellery have in their mouth more mutans streptococci (p=0,032) and also more lactobacilli (statistically unsignificant) than subjects without tongue jewellery. The former also have greater index of visible plaque (VPI) (p=0,009) and gingival bleeding index (GBI) (statistically unsignificant) as well as greater index of decayed surfaces (DS) and decayed, missing and filled surfaces (DMFS) as subjects without tongue jewellery. Conculsions There is a voltage between amalgam fillings and tongue jewellery and occasionally a current if there is a contact between amalgam fillings and tongue jewellery. In contradiction with hypothesis subjects with tongue piercing have in their mouth more mutans streptococci and also lactobacilli than subjects without tongue piercing. The difference can be conected to VPI and also DS, DMFS and GBI, but not statistically significant.
